Francis, a young man, recalls in his memory the horrible experiences he and his fiancée Jane recently went through. It is the annual fair in Holstenwall. Francis and his friend Alan visit The Cabinet of Dr. Caligari, an exhibit where the mysterious doctor shows the somnambulist Cesare, and awakens him for some moments from his death-like sleep. When Alan asks Cesare about his future, Cesare answers that he will die before dawn. The next morning Alan is found dead. Francis suspects Cesare of being the murderer, and starts spying on him and Dr. Caligari. The following night Cesare is going to stab Jane in her bed, but softens when he sees the beautiful woman, and instead of committing another murder, he abducts her. Jane's father awakens because of the noise, and he and some servants follow the fleeing Cesare. When Cesare cannot outrun his pursuers anymore, he gently places Jane down on the ground...

Il film inizia con un racconto di Franz (Friedrich Fehér): un signore poco raccomandabile (il dottor Caligari, Werner Krauss) giunge alla fiera del paese per presentare il suo sonnambulo, Cesare (Conrad Veidt). Si dice che il sonnambulo, una volta svegliato, predica il futuro... Contemporaneamente al suo arrivo cominciano ad avere luogo nel paese delle morti sospette.
Il primo a chiedere una predizione è l'amico di Franz, Alan (Hans Heinrich von Twardowski), che con Franz condivide l'amore per Jane (Lil Dagover). Cesare predice ad Alan che morirà entro il mattino seguente e la predizione si avvera.
Altre morti inquietanti seguono a questa. Franz, nel tentativo di smascherare il colpevole, scopre che il dott. Caligari controlla il sonnambulo ordinandogli di compiere quei cruenti omicidi.
Alla fine del racconto Franz accusa il direttore del manicomio in cui é rinchiuso, il Dr. Oscar, di essere il Dr. Caligari. In realtà si scopre che è proprio Franz ad essere pazzo, e che probabilmente tutto il racconto è frutto di una sua allucinazione. Jane è una delle pazienti ed il Dr. Caligari (nella realtà Dr. Oscar) è effettivamente il direttore del manicomio.
